Please read these Terms & Conditions carefully before purchasing Playhouse Raffle tickets. By purchasing tickets, you agree to be bound by these Terms & Conditions.
1. Sponsoring Organization
The Playhouse Raffle is sponsored by the Riverside Healthcare Foundation, located at 350 N. Wall Street, Kankakee, IL 60901. Raffle License No. [INSERT LICENSE NUMBER], issued by [INSERT ISSUING MUNICIPALITY/COUNTY].
2. Purpose
All net proceeds from the Playhouse Raffle benefit Riverside's Behavioral Health Unit Redesign & Modernization Project, supporting the creation of a more modern, therapeutic, and healing environment for patients and families receiving inpatient behavioral healthcare.
3. Prizes
Two (2) playhouses are being raffled. Each playhouse has its own separate raffle and its own winner.
- Playhouse 1 — Johnson-Downs Construction Playhouse: Backyard Adventures Olympian Outlook XL 2B Wooden Swing Set. Suggested play area: 25' × 33'.
- Playhouse 2 — Glade Plumbing & Piping Playhouse: Outdoor Living Today Cozy Cabin Playhouse. Dimensions: 90" L × 108" W × 157" H.
Prizes are donated by community partners. No cash substitutions or alternative prizes will be awarded. Prizes are non-transferable.
4. Ticket Pricing
- $5.00 per ticket (per raffle)
- 5 tickets for $20.00 (per raffle)
Participants may purchase tickets for either raffle or both raffles. Tickets for each raffle are sold and tracked separately.
5. Eligibility
- Participants must be 18 years of age or older at the time of purchase.
- Participants must agree to these Terms & Conditions at the time of purchase.
- Employees and board members of Riverside Healthcare Foundation and immediate family members residing in the same household are not eligible to participate.
- Winners must meet all delivery and site-access requirements outlined in Section 8 to claim the prize.
6. How to Enter
Tickets may be purchased online at www.riversidehealthcare.com/playhouse using credit card or ACH payment. At the time of purchase, participants must acknowledge they have read and agreed to these Terms & Conditions.
7. No Purchase Necessary — Alternate Method of Entry

A common approach: participants may enter by mailing a written request with their name, address, phone number, and the name of the raffle (Johnson-Downs or Glade) to the Foundation by [DATE]. One free entry per envelope per raffle. Free entries have the same odds as purchased entries.]
8. Drawing
- The drawing for both raffles will take place on July 10, 2026.
- One winner will be selected for each raffle through a random drawing process.
- The drawing will be conducted by Riverside Healthcare Foundation and will be livestreamed on Riverside Healthcare social media channels.
- If the same individual is selected as the winner of both raffles, they will receive the first playhouse drawn and a new winner will be selected at random for the second raffle.
- Odds of winning depend on the total number of eligible tickets sold for each individual raffle.
9. Winner Notification
- Winners will be contacted using the contact information provided at the time of ticket purchase.
- Winners must respond to the Foundation by 5:00 p.m. on Monday, July 13, 2026.
- If a selected winner cannot be reached or does not respond by the deadline, that individual will forfeit the prize and a new winner will be selected at random from remaining eligible entries.
10. Delivery Requirements
- The delivery location must be within a 30-mile radius of Riverside Medical Center, 350 N. Wall Street, Kankakee, IL 60901.
- Winners do not need to reside within the delivery radius; however, the playhouse delivery location must fall within the delivery radius.
- Delivery is anticipated between July 14–17, 2026. The exact delivery date and time will be coordinated with the winner during normal business hours.
- The winner is responsible for ensuring adequate yard access for delivery, including but not limited to: sufficient gate clearance, access to the delivery area, level or reasonably graded ground, and any required site preparation.
- Winners are responsible for any required anchoring, installation, or permit requirements that may apply in their municipality.
- Failure to meet delivery or site-access requirements by the coordinated delivery date will result in forfeiture of the prize and selection of an alternate winner.
11. Forfeiture
A prize will be forfeited and a new winner selected if the selected winner:
- Does not respond by 5:00 p.m. on July 13, 2026;
- Does not meet eligibility requirements;
- Cannot accept delivery within the delivery window and delivery radius;
- Does not meet site-access requirements for delivery; or
- Declines the prize.
12. Taxes and Prize Reporting
Winners are responsible for all applicable federal, state, and local taxes associated with prize receipt. Prizes with a fair market value of $600 or more may be subject to IRS reporting requirements. The Riverside Healthcare Foundation may be required to issue a Form 1099-MISC to the winner. By accepting the prize, the winner consents to the use of their name and contact information as required for tax reporting purposes.
13. Publicity
By participating in the Playhouse Raffle, winners consent to the use of their name and general location (city/state) for promotional purposes in print, digital, and social media communications by the Riverside Healthcare Foundation and Riverside Healthcare, unless prohibited by law.
14. Limitation of Liability
The Riverside Healthcare Foundation and Riverside Healthcare are not responsible for any injuries, losses, damages, costs, or expenses arising from participation in the raffle or acceptance, possession, or use of the prize. The Foundation reserves the right to cancel, modify, or terminate the raffle at any time if, in its sole judgment, the raffle cannot be conducted as planned due to circumstances beyond its control, including but not limited to technical failure, natural disaster, or acts of God.
15. Governing Law
This raffle is governed by the laws of the State of Illinois, including the Illinois Charitable Games Act (230 ILCS 30), and is subject to all applicable local ordinances and regulations.
